Hmm. I very much like the motif of Itachi sexually abusing Sasuke to make him hate him more. Adding waterboarding to the mix was a nice twist. The beginning was good, with Sasuke being cruelly dragged and thrown, I liked how you did that. During the lemon, I think you should have emphasized Sasuke's pain more and his pleasure less, since any pleasure he might have been feeling from his penis and prostate being stimulated would have been trivial compared to the combined agony of anal violation and suffocation.
